In that case you (probably) did it online.
You download the PDF of your tax return as follows:
Go to My Tax Authorities and log in with your DigiD.
On the welcome page, click on 'Correspondence'.
In the overview you can find the declaration you need. With the income tax filter you can search for specific years. The message you need has the subject: 'Income tax return <year>'.
Click on the link and you can download the PDF and save it to your computer and send it to us.
